Indications/Uses
Management of angina pectoris, hypertrophic obstructive cardiomyopathy, essential tremor; control of HTN & most forms of cardiac arrhythmia; long-term prophylaxis against reinfarction after recovery from acute MI; relief of situational anxiety & generalised anxiety symptoms, particularly those of somatic type; adjunctive management of thyrotoxicosis & thyrotoxic crisis; in combination w/ an α-blocker in management of phaeochromocytoma peri-op; prophylaxis of migraine & upper GI bleeding in patient w/ portal HTN & oesophageal varices.
Dosage/Direction for Use
Adult & childn ≥12 yr Angina, migraine & tremor Initially 40 mg bd-tds; then 120-240 mg/day for angina; 80-160 mg/day for migraine & tremor. HTN Initially 80 mg bd, then 160-320 mg/day. Arrhythmia, anxiety tachycardia, hypertrophic obstructive cardiomyopathy & thyrotoxicosis 10-40 mg tds-qds. Post MI Initially 40 mg qds for 2-3 days, then 80 mg bd. Phaeochromocytoma Pre-op: 60 mg daily for 3 days. Treatment dose: 30 mg daily. Anxiety 40 mg before anxious situations; 40 mg bd-tds for long-term anxiety. Portal HTN Initially 40 mg bd, may be increased to 80 mg bd; max: 160 mg bd. Childn <12 yr Migraine 20 mg bd-tds. Doses are decided using body wt in all other indications.
Administration
Should be taken on an empty stomach.
Contraindications
Hypersensitivity. Patients w/ untreated/uncontrolled heart failure, shock; heart conduction or rhythm problems; slow heart rate; low BP; severe blood circulation problems; DM; metabolic acidosis; strict fasting diet; Prinzmetal’s angina; asthma; untreated phaeochromocytoma.
Special Precautions
History of allergic reactions; myasthenia gravis; heart weakness or 1st degree heart block; blood circulation problems; symptoms of hyperthyroidism; psoriasis; Raynaud’s disease or intermittent claudication; smoking. Renal or hepatic impairment. May impair ability to drive or use machinery. Sugar intolerance. Avoid alcohol intake. Concomitant use w/ anaesth. Abrupt discontinuation. Pregnancy & lactation.
Adverse Reactions
Tiredness, cold extremities, difficulty in sleeping; slow or irregular heartbeat, Raynaud's syndome; nightmares.
Drug Interactions
Verapamil, diltiazem; disopyramide, quinidine, amiodarone; ergotamine derivatives; adrenaline; anti-diabetics including insulin; lidocaine, propafenone, flecainide; indometacin; digitalis glycosides; chlorpromazine; cimetidine; α-blockers, clonidine, moxonidine, methyldopa or hydralazine; MAOIs, imipramine or fluvoxamine; warfarin; rizatriptan; rifampicin; barbiturates; theophylline; diuretics.
